When registering for seminars, conferences, etc. with a credit card, please be sure to include the following information:

* Type of credit card used (i.e., MC, VISA, American Express, Discover).
* Account number on credit card..
* Expiration date on credit card.
* Amount being charged.
* Cardholder's address, including zip code.
* Cardholder's phone number.
* Signature (if a registration form is used).
* Receipt for credit card transaction (yes or no).
